Insurance and Payment Policies

The insurance maze

Understanding and complying with Insurance is a maze for clients and providers alike. To best support you as a client or potential client, we’re providing the following information so you can gain a better understanding of how our practice operates, as well as some guiding insights about working with your insurance.

Cost per session:

Therapy at Eagle Rock Therapy averages $150 per session hour (50 minutes). We will work on a sliding scale depending on the individual’s income.

If you are seeking reimbursement from your insurance provider, it is our policy that we charge each session and will provide you as our client a statement that you then will submit for reimbursement.

Payment Methods

Cash-paying clients can pay by debit or credit cards.

Credit card information is kept confidential, even from the provider. Your information is never disclosed.

A client’s payment card is charged at the end of the session if paying without insurance. Insurance copays are charged at the end of each session.

Cancellation Policy

You must notify Eagle Rock Therapy of your intent to cancel your session at least 48 hours in advance of your appointment. Failure to do so will result in the full charge of your session hour.

Our therapists will wait a maximum of 20 minutes before it is considered a no-show, at which time you will be charged the full session rate. Insurance will not cover your missed sessions.

Providers We Work With

Medi-Cal/Medicaid

We accept Medicaid through Los Angeles County only.

To verify your insurance you can emaildamarisrmartinez@gmail.com with your subscriber ID or name and birthdate and she will search to confirm your benefits are active.

It is important to renew your coverage in a timely manner. Each year there are changes at the beginning, end and during special enrollment periods for Medicaid clients/subscribers.

As a provider, we are required to stop services immediately in the case of lapses in coverage.

Health Net MHN

Our associate therapists (working under licensed supervision) accept Health Net MHN for Medicaidonly. Health Net is a medical underwriter that then provides the mental health needs of a plan to various mental health companies such as MHN or Molina.

If you have Health Net Molina and not MHN, you can call Health Net atHealth Net Medi-Cal Member Services 1-800-675-6110 option 1.  They are familiar with the request to switch mental health benefits from Molina to MHN.

Medicaid Health Net MHN benefits include:

  • no copay
  • 52 weekly sessions per yearÂ
  • no referral necessary
  • extra sessions available as long as medically necessary for example in a crisis.
  • they cover EMDR and IFS (trauma therapies)
  • online sessions are covered

LA Care Health Plan

Licensed therapists accept the following insurances through a company called Headway:

  • Healthnet/MHN
  • Aetna
  • Anthem Blue Cross California
  • Oxford
  • Blue Shield of California
  • Oscar Health
  • United
  • Cigna
  • Magellan
  • Kaiser Permanente of Northern California

If using Headway and an insurance provider listed above, it is crucial to call the behavioral health number on the back of your card to confirm your benefits, copayments, coinsurance and deductible, if any.Headway may take 2 weeks or more to process your application.

An important note

All of the above insurance providers have a variety of plans, all with different benefits. You are responsible for familiarizing yourself with the treatment options available to you. Your provider’s client services or member services department will be able to confirm your benefits and options based on your plan.

We do our best to avoid any complications or disruptions to you receiving care with Eagle Rock Therapy. It would be unfortunate for all if a service we provide is not covered by your insurance.

Caveats – we hate to be the bearer of bad news but…

No insurance plan is the same. Even if we accept the insurance you carry, we might not work with your plan. Please do your due diligence and become familiar with your insurance when seeking therapy at Eagle Rock Therapy.

Doing Good for Our Clients

If cash-paying clients decide to pay more than the amount they’re quoted, the difference will be used to help less fortunate clients receive therapy.
